We are pleased with the opportunity to be working with your company. If you are supplying labor and/or materials for Apex Restoration you will be required to carry all items listed below. 

  • A Signed Copy of the Subcontractor Agreement (Link listed below as MSA)

  • A Current General Liability Certificate (Minimum 1,000,000.00 each occurrence 2,000,000.00 aggregated) 

  • A Current Workers Compensation Certificate or a Coverage Waiver (see below) 

  • A W-9 for taxpayer identification 

  • A Copy of Appropriate Business or Contractor License 

We need to have all current information on file before any payments are issued. We pay 30 days from when the invoice is received in our office (please be aware we process checks weekly so give or take a few days). Please email invoices to invoicing@apexrestoration.pro. Please understand that Apex Restoration will not pay any invoice for work performed until we have received all required documentation. As a reminder it is up to the subcontractor to make sure you send new documents when they expire. Please have your insurance carrier list Apex Restoration as a certificate holder (see below) on your insurance policies so we may continue to receive updated policy information. (Please follow up with your insurance carrier so that they send current certificates.)

Please list Apex Restoration as an additional insured on all jobs. Email to invoicing@apexrestoration.pro. Workers Compensation Coverage Waiver – FOR SUBS WITH NO EMPLOYEES We do require ALL subcontractors that do not have employees to carry a workers compensation exclusion policy. This exclusion policy waives any rights under the Utah Workers Compensation Act for your company’s owner, partners, or corporate officers and releases Apex Restoration from any liable injury to those individuals. If you have a waiver policy, please send a copy of the letter from the Utah Labor Commission that shows effective dates. If you do not have a waiver policy then you may obtain one from the Utah Labor Commission of Utah. The cost is $50 every year www.laborcommisison.utah.gov. Please be advised that they do not send out renewal reminders so make sure you keep track.
